AID to PUV DRIVERS
Project Description
The assistance for public Vehicle drivers is intended to all drivers without license in the municipality who are driving PUV vehicles like tricycles, jeepney, multicab and van. Driver's license is important for them to legally drive or operate a motor vehicle. As a part of employment opportunity, they can go around in their route or jurisdiction without hesitation that they can be caught by the authority for having violations for not having a driver's license. Applicants can avail of assistance in two (2) ways:
a) Assistance in Obtaining Professional Drivers License
b) Assistance in the Renewal of Professional Drivers License
Qualification
A. Assistance in Obtaining Professional Driver License
Eligibility Requirements

A bona fide residents of Bansud

Must be of Good Moral Character certified by the Barangay Captain

Must be physically and mentally fit
-
Must be PUV driver duly certified by cooperative, association where he is a member and or owner of the vehicle
Requirements for Application

Accomplished Application Form
-
Barangay Clearance and Certificate of Indigency
-
Photocopy of Registration or Mayor's Permit of Vehicle
-
Photocopy of Student Permit
-
Photocopy of Professional Drivers License (new)
-
Certification as PUV Driver
Requirements in the Application of New Professional Driver's License (LTO)
-
Duly Accomplished Application for Driver's Licenses (ADL)
-
Valid Student Permit (at least 6months old)

Medical Certificate from LTO Accredited Physician

Tapayer's Identification Number
-
Clearances (NBI or Police Clearance)

Must have passed the written and practical examinations
-
Must not have been cited for (2) or more counts of reckless driving during the period of validity of his existing license
B. Assistance for Driver's License Renewal

Current driver's license (expired for not more than 2 years)

Electronically transmitted medical certificate in LTO prescribed form.
